Data Nurse Data driven district nursing care

District nursing care is under increasing pressure. Promoting independent functioning in patients can ease this pressure, but nurses lack adequate support in achieving this. Data Nurse provides data-driven support to nurses by utilizing insights from electronic health records to enhance the patient’s independent functioning and improve care quality.

Objective

Data Nurse is working on setting up a learning healthcare system where healthcare professionals in district nursing care can learn from registered care data to enhance the independent functioning of their patients. Patients, caregivers, nurses, and researchers are collaborating in this project to enable people to live independently at home for as long as possible.

Results

  • Identifying the preferences and needs of professionals for working with data.
  • A nursing dashboard focused on practice, able to monitor patients’ independent functioning and provide suggestions for improvement.
  • A shared decision-making tool that allows patients to indicate their own (care)goals.
  • A learning healthcare system where professionals can learn from both the data and each other to improve care

Approach

Preferences and needs are mapped out through focus groups and interviews. Additionally, a shared decision-making tool for community nursing is specified, and a dashboard is developed to enable learning from data. Then, a complex intervention is developed in which shared decision-making and learning from data are important components. All elements come together in a pilot study to study the effectiveness of the intervention.

Impact on education

In this project, developed E-learnings and training sessions are made available for clinical practice and education. We collaborate with organizations from both practice and science. The results are also publicly shared through publications. The Universities of Applied Sciences in Utrecht and Amsterdam integrate the findings into their curricula. Finally, students are involved in data collection and evaluation.
